The use of sphincterotome in ERCP
There are two main applications of the sphincterotome in therapeutic ERCP:
1. Expand the duodenal papilla sphincter to assist the doctor in inserting the catheter into the duodenal papilla under the guidance of the guide wire.
The incision-assisted intubation here is mainly reflected in the “bow” and “toughness” of the sphincterotome. 2. Cut the duodenal papilla sphincter to enlarge its opening to meet subsequent treatment needs such as removal of common bile duct stones.
The use of the sphincterotome here is mainly to incise the bile duct sphincter, that is, the inner section of the duodenal wall of the Oddi’s sphincter, so the length and size of the incision are appropriate to avoid perforation and to meet subsequent treatments such as stone removal.

The arc design formed by pulling the wire makes up for the angle and field of view of the side mirror (duodenoscope) when intubating the bile and pancreatic ducts. This enables the operating doctor to well control the direction of the sphincterotome to the predetermined 11 o’clock.
2. Needle-shaped knife, also known as “needle knife”. It is an important supplement when using arcuate sphincterotome clinically. The tip of the needle-shaped knife is a needle-like wire, which not only has the function of piercing, but also has the toughness required for electrocution and “cutting”. At present, the needle knife is mainly used in pre-incision to assist in the intubation process of nipple stone incarceration and difficult nipple intubation.
